The post holder will support the Federations Service Lead and PCN Manager with the continued implementation of the nationally recommended Personalised Cancer Care Programme (Holistic Needs Assessment & Care Planning, Health and Well-being Event, Treatment Summary, Self-Management including risk stratified remote monitoring and surveillance).
